CircuCore Technologies ("CircuCore," "we," "us," or "our") is a durable medical equipment (DME) provider. This Privacy Policy explains what information we collect from and about the patients we serve, how we use it, how we protect it, and the choices you have. CircuCore handles protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law.
Information we collect
We collect information needed to verify your eligibility, fulfill your device order, bill your insurance or process self-pay, and keep you informed about your order. This includes:
- Identity and contact information — your name, date of birth, mailing address, email address, and mobile phone number.
- Health information — your prescription/order, referring provider, diagnosis codes, clinical notes, and related documentation provided by your healthcare provider.
- Insurance information — your insurance carrier, policy and group numbers, and coverage details.
- Payment information — for self-pay orders, payment details processed through our payment processor.
- Documents you or your provider submit — such as the prescription, insurance card, and (if provided) a photo ID.
How we use your information
- To verify insurance coverage and obtain any required authorization.
- To process, fulfill, and ship your device order.
- To submit and manage insurance claims and billing, or process self-pay.
- To send you service notifications by email and text message about the status of your own order (see below).
- To provide customer support and respond to your questions.
- To comply with legal, regulatory, and HIPAA obligations.
Text messaging and mobile information
Patients are referred to CircuCore by their healthcare provider, who shares the patient's contact information — including a mobile phone number — with us so that we can fulfill and service the device order. We treat your mobile number as contact information.
We do not add you to our text-message program automatically. Before sending any order-update texts, we send you a single one-time welcome message asking whether you would like to receive text-message updates about your order. We send further text messages only if you confirm — for example, by replying YES. If you do not confirm, we will not text you and will keep you informed by phone, email, or mail instead. Receiving text messages is optional and is never a condition of receiving your device or any other service.
If you opt in, we use your mobile phone number solely to send you transactional service notifications about your own device order, as described in our Terms of Service.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent are never sold or shared with any third parties.
You can opt out of text messages at any time by replying STOP to any message.
How we share your information
We do not sell your personal information, and we do not share it for third-party marketing. We share information only as needed to provide our services and as permitted or required by law, including with:
- Service providers who process information on our behalf — for example, secure cloud hosting, text-message and email delivery, shipping carriers, and payment processing. These providers are bound by contract (including HIPAA Business Associate Agreements where applicable) to protect your information and use it only to perform services for us.
- Your insurance carrier and related payers, to verify coverage and submit claims for your device.
- Your healthcare provider, in connection with your referral and care.
- Authorities, when required by law, regulation, legal process, or to protect rights and safety.
How we protect your information
We maintain administrative, technical, and physical safeguards designed to protect your information, including access controls, encryption in transit, and audit logging of access to protected health information.
Your choices and rights
You may opt out of text messages at any time (reply STOP). As a patient, you also have rights regarding your protected health information under HIPAA, including the right to access and request corrections to your records. To exercise these rights, contact us using the details below.
